Supplementary Health Agency, which approved the recognition of noncompliance with the second (§ 2°, § 3º, § 4º and § 5º) and third clauses of the Commitment Agreement (Termo de Compromisso) by the operator BIO
SAÚDE SERVIÇOS MÉDICOS LTDA, registration 40296-6, and the application of the fine provided for in the amount of R$1,200,000.00 (one million two hundred thousand reais), in addition to declaring the termination of the Commitment Agreement
